Clutch Issue?

At Turn3 Autosport we replace clutch kits regularly. We have seen more than we can count. However, we have never seen one that looks like this. Car came on a flatbed with the clutch pedal on the floor…Clutch slave was leaking and car had a distinctive burnt clutch smell. Once a new slave was installed the car made a grinding sound when the clutch was depressed…

So, what happened first? Which part was the first to fail?
